And what about all the others, like the handlers, the Agency staff, Mario Bossi, Pino and so on? Would they correspond to real world people, or just be elements of the story, like Glenn Scott's Wise Man?

Well if they were in the hospital, then the Medical Branch would be the doctors. Lorenzo could be the Hospital Administrator. The handlers be physical therapists (hence the "training") and Ferro, Priscilla and Olga could be nurses or staff.

I dunno, I think it'd be an interesting twist, following the hospital line of thought, if Dante turned out to just be some dumb schmuck who wound up in the wrong place at the wrong time. To take an entirely unoriginal path: car-crash (or considering the number of girls; bus crash) and Dante was the drunk who caused it... and how has to live with the guilt of that or is trying to make amends...

...now I'm going to ramble out loud for a bit:

Say "Dante" had something or a combination of things go downhill for him (company failure, lost his wife, lost his kid... whatever). Got hammered as a way to deal with it (bad decision, but I'm sure we've all really needed a drink at one point or another) tried to drive home (even dumber idea... but a good man with a momentary lapse of discretion would be a far more tragic twist in the tale) and ran a bus carrying the girls in it off the road. Now he's up on manslaughter, or even has been in gaol for the offence and when he gets out is when he arrives in the story...

...Who knows: maybe he's even Pagani rich and is actually the one funding the girls' treatment.

Disregard what I said earlier; this is far better. It even sort of explains why Angie's 'dream backstory' involves an auto accident.

I can picture the climactic scene now. The girls' dream sequences, unfolding lucidly with their actual selves acting the scenarios out in the physical world, culminate in them slipping out of the hospital, hunting Dante down and attempting to unleash bloody vengeance... only to discover that the poor wretch is, in real life, saddled with agony far worse than they could hope to inflict - the torment of a guilty soul, reduced to a neurotic wreck and forced to stifle the pain via a daily cocktail of alcohol and drugs that drags him ever closer to an early grave.

that was brilliant in the most terrifying way possible. Maybe when they sing that song is when they realize that they are in a shared lucid dream. Fack it, toss in inception too, new dream therapy and that is their wakeup call. They all wake up in one big hospital room and collectively WTF, haha.
